WHAT THE PROJECT DOES
The project provides a bespoke, person centred, coordinated package of support alongside a personalised budget that is responsive to the identified needs and service-user choice of the beneficiaries.
The key feature of this project is the focus on the economics of working with individuals with multiple needs. The project endeavours to make a case for the sustained investment in effective interventions for individuals with multiple needs. In order to do this it will translate outcomes and cost data into an economic case for change by undertaking the following:
- Developing an infrastructure to purchase services using personalised budgets where gaps in local services exist.
- Providing a service delivery team, You First, that will act as ‘skilled negotiators’ on behalf of the project beneficiaries to assist them in navigating through current service provision.
- Developing a system-wide commissioning map to provide an overview of how resources are currently used and their effectiveness in delivering outcomes at present. This will be combined with local needs/prevalence data with measured outcomes from the project to model how resources could be used different to achieve better outcomes.
- Individual personal histories will be used to cost prior service use to identify where effect preventative interventions may be possible at an earlier stage in the recovery journey.
